Type 2 Diabetes 8 week program facilitated by our Exercise Physiologist and Dietician for healthy eating, physical activity and peer support. Participants are assessed prior to commencement and again after the 8 week program.

Cost: $20.00 Medicare rebate available out of pocket $3.05
Initial individual assessment to determine eligibility will be provided before the program commences. This Assessment will take 45mins, cost $93.75. Medicare rebate is available, out of pocket cost for the assessment will be $40.80.
